    You must contact your instructors and get emailed approval to attach to this form prior to submission. If you do not already have approval from your instructors, do not begin completing this form until you have proof of approval that you can attach and upload.

    If you are late registering for multiple classes, you must submit all courses and instructor approvals on a single form. Submission of multiple/duplicate forms will delay processing and may cause your form to be rejected.

        • requires a $100.00 late fee
        • requires instructors' approval
        • requires academic dean's approval (College Dean of your academic major)
        • will be cancelled if required payment is not received on the same day
